Northgate Health Care Facility in North Tonawanda, NY, seeks an LPN Supervisor to lead nursing staff and ensure high-quality resident care. You will oversee clinical operations, administer medications, and maintain regulatory compliance in a fast-paced skilled-nursing setting.

The role offers a full-time overnight schedule (11 pm–7 am), with a $32–$34/hour base pay, plus a $3.00 overnight differential and a sign-on bonus up to $3,000.

Qualifications

• LPN license valid for New York state is required.
• High School Diploma or equivalent required; Associate's degree preferred.
• Current CPR certification required.
• Supervisory or leadership experience preferred.
• Experience in long-term care or skilled nursing preferred.
• Strong clinical assessment, communication and organizational skills.
• Ability to lead, mentor, and support nursing staff.

Responsibilities

• Supervise nursing staff and support the delivery of high-quality resident care.
• Administer medications, treatments, and nursing services per physician orders and policies.
• Monitor resident conditions and ensure interventions, documentation, and communication.
• Collaborate with residents, families, physicians, and interdisciplinary team members on care plans.
• Provide coaching, training, and mentorship to nursing staff.
• Ensure compliance with state and federal regulations and infection control.
• Maintain accurate documentation of resident care and outcomes.
• Address clinical concerns and staffing issues to ensure continuity of care.
• Perform additional duties as assigned.

Rate: $32- $34 (depending on experience)
Shift Differential: + $3.00 extra for overnight

RESPONSIBILITIES:

• Supervise nursing staff and support the delivery of high-quality resident care

• Collaborate with residents, families, physicians, and interdisciplinary team members to support individualized care plans
• Provide coaching, training, and mentorship to nursing staff to promote professional growth and teamwork
• Ensure compliance with state and federal regulations, infection control practices, and facility policies
• Maintain accurate documentation of resident care, treatments, and clinical outcomes
• Address clinical concerns, staffing issues, and resident care needs to ensure continuity of care
• Perform additional duties as assigned

REQUIREMENTS:

• High School Diploma or equivalent required; Associate's Degree preferred
• Valid NYS Licensed Practical Nurse (LPN) License required
• Current CPR Certification required
• Supervisory or leadership experience preferred
• Experience in long-term care or skilled nursing preferred
• Strong clinical assessment, communication, and organizational skills
• Ability to lead, mentor, and support nursing staff
• Strong problem-solving abilities and attention to detail
• Commitment to providing compassionate, resident-centered care
