Job Summary

The LPN is responsible for providing direct nursing care to the residents in an assigned unit under the direction of the shift supervisor and supervises the day-to-day nursing activities performed by the nursing assistants.

Responsibilities:
  • Is knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all residents in a safe, secure environment
  • Supports and cooperates with specific procedures and programs for:
  • Safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs
  • Confidentiality of all data including resident, employee and operations data
  • Quality Improvement and compliance with all regulatory requirements
  • Compliance with current law and policy to provide a work environment free from sexual harassment and all illegal discriminatory behavior
  • Supports and participates in common teamwork:
  • Cooperates and works together with all co-workers
  • Use tactful, appropriate communications in sensitive and emotional situations
  • Follows up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems and concerns
  • Completes requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned
  • Give and receive condition report from the on-coming and off-going shift
  • Make rounds and routine checks on patients/residents
  • Observe and report symptoms and conditions of residents/patients
  • Administer treatments and medications as prescribed by a physician
  • Maintain records reflecting resident condition and treatment including nurse’s notes, discharge plans, resident assessments, doctor’s orders, etc.
  • Notify physicians when resident’s condition warrants
  • Admit, transfer and discharge residents according to policy.
  • Review, monitor, intervene and report complaints/grievances made by residents, families, etc.
  • Perform emergency procedures such as Cardiopulmonary Resuscitation (CPR) and report accidents or incidents in a timely manner
  • Assign duties to non-professional nursing personnel and supervise and evaluate work performance
  • Monitor and update care plans for each shift as appropriate
  • Maintain a physical, social and psychological environment which is conducive to the interest and welfare of the residents.
  • Investigate and report employee incidents according to company policy and coordinate return to work programs with Human Resources
  • Participates in appropriate meetings and training
Qualifications:
  • Current, unencumbered license to practice as an LPN in this state.
  • IV Therapy 30-hour Certification Current CPR Card
  • Minimally two years as an L.P.N., preferably in a Long-Term Care or Hospital environment.
  • Able to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
  • Able to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage and to draw and interpret bar graphs.
  • Basic computer skills and ability to use nursing programs, email, and other online reporting programs.
  • Able to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
  • Able to perform tasks which may be physically demanding such as pushing, bending and lifting up to 60 pounds.
  • Able to maintain positive working relationships with residents, their families, peers and other team members.
  • In good physical and emotional health and free of communicable diseases
  • Able to perform frequent lifting, pushing, pulling, moving of equipment, bending, and stooping.
  • Able to spend long periods of time on foot.
