Qualifications

• Active New Jersey LPN license.
• Previous nursing experience as an LPN.
• Experience in long-term care or subacute rehab preferred.

Responsibilities

• Monitor residents' condition and report changes to the RN.
• Handle admissions, discharges, and transfers.
• Provide hands-on nursing care per each resident's care plan.
• Administer medications and treatments and chart on time.
• Contribute to care planning and conferences.
• Support residents and families and collaborate with the care team.
