Job summary

Rocky Mountain Care Hunter Hollow in West Valley City, UT seeks a Licensed Practical Nurse (LPN) to join the Nursing Services team. The role involves providing direct nursing care, administering medications, and coordinating with CNAs and other staff in a busy skilled-nursing environment.

Current LPN license, CPR certification, and long-term care experience preferred. This on-site role reports to the Director of Nursing and offers stable hours with competitive hourly pay of $30.00 to $36.00.

Qualifications

• Prior experience in long-term care preferred but not required.
• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.

Responsibilities

• Directs the daily activities of the licensed practical/vocational nurse and certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Follows the nursing model to promote holistic care for the residents.
• Ensures that policies and procedures are complied with by nursing personnel assigned.
• Participates in the admission, discharge and transfers of residents as required.
• Transcribes physician orders to medical record and carries out orders as written.
• Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated.
