Qualifications

• Experience in Skilled Nursing/Rehabilitation facilities preferred.

Responsibilities

• Assist in implementing individualized treatment plans for residents.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services to residents and patients.
• Evaluate and document residents' medical status on admission and during treatment.
• Perform other duties as assigned.
