Senior Resource Group-SRG is seeking an LPN/RN for our Scottsdale senior living community. The nurse will provide nursing services, administer medications, and oversee the Medication Management Program consistent with physician orders and state regulations.

Qualifications

• Must be a Licensed Practical Nurse and/or Registered Nurse holding a valid nursing license in good standing.
• Minimum of one (1) year of experience in a senior living environment preferred
• First Aid and CPR certification required
• Must pass a criminal background clearance before beginning work
• Proficient with Microsoft Office, eMAR and other related software

Responsibilities

• Work as part of the team within the Health Services Department
• Participates in and completes resident’s assessments to identify resident care needs
• Develops and updates resident service plans which include person centered approaches and interventions to determine resident care level
• Follows up on residents’ health conditions, and /or changes of condition with healthcare providers
• Communicates relevant resident information to healthcare providers and alerts staff of care concerns and service plan changes
• Performs nursing services as directed and in accordance with the Nurses Practice Act and state regulations
• Develops and maintains relationships with residents, families, and other healthcare providers
• Collaborate with healthcare providers by reviewing weekly visit notes and ensuring continuity of care
• Assists with coordination of care for new move-ins and residents returning from leave or new provider services
• Completes documentation via Yardi EHR according to SRG policy and state regulations
• Initiates internal incident reports and participates in investigations as needed
• Coordinates residents’ incidental medical, dental, vision, hearing, podiatry care and telehealth visits
• Coordinates onsite lab testing and durable medical equipment as needed
• Completes Long Term Care insurance verification forms
• Supports Medication Management Program with focus on med room systems and storage
• Collaborates with preferred pharmacy partners
• May provide ADL care and assistance with medication administration as needed
• Assists Director of Assisted Living with staffing issues as needed
