The role includes medication management, continuous resident assessment, and collaboration with the healthcare team to ensure safety, wellness, and accurate documentation. A positive, flexible team player willing to work weekends is essential.

Qualifications

• Current LPN license to practice in the state where the community is located.
• Experience with the senior population, preferably in assisted living or LTC.
• Must be a team player who works well with others and has experience supervising staff.
• Must possess critical thinking skills, ability to effectively communicate verbally and in writing.
• Demonstrated ability to manage time efficiently with strong organizational skills.
• Must be in good physical condition and able to lift up to 50 lbs.
• Must be willing to work an 8-16 hour flexible schedule including weekends and holidays.
• Must attend required trainings, CEUs and staff meetings.

Responsibilities

• Execute the medication management plan with periodic review and physician communication when changes are needed.
• Continuously assess resident needs and update care and services accordingly.
• Implement measures to ensure resident wellness and safety with proper documentation.
• Collaborate with the healthcare team to meet resident needs.
• Respond to emergencies in a timely, calm, and competent manner.
• Report incidents and changes in resident condition with appropriate documentation.
• Assist with meals and monitor resident nutrition and acceptance of diets.
• Maintain high standards for record keeping and documentation across departments.
