Job summary

Harmony Senior Services in Martinsburg, WV is seeking a qualified Licensed Practical Nurse to support resident care and medication management. You will collaborate with the healthcare team, ensure safety, and document changes in residents' conditions and care plans.

Ideal candidates have senior care experience, strong communication, and the ability to supervise staff while maintaining compliance with state requirements. Flex scheduling and CEU attendance are expected.

Qualifications

• Must hold a valid LPN license in the state of practice with good standing.
• Experience with senior residents, preferably in assisted living or LTC.
• Ability to supervise staff and work in a team.
• Demonstrated critical thinking and strong written/verbal communication.
• Good time management and organizational skills.
• Must be physically able to lift up to 50 lbs and perform transfers.
• Willing to work a flexible 8–16 hour schedule, including weekends/holidays.
• Attend trainings/CEUs and comply with state certification requirements.

Responsibilities

• Execute and review the medication management plan and coordinate with physicians as needed.
• Assess resident needs and update care plans accordingly.
• Ensure resident safety and proper documentation of measures taken.
• Collaborate with the healthcare team to meet resident needs.
• Respond to emergencies promptly and calmly.
• Document incidents and changes in resident condition with the Healthcare Director.
• Participate in community activities and events.
• Assist with meal delivery and monitor diet acceptance to ensure nutrition.

Job Description

• Execution of the medication management plan including periodic review of the documentation, observation of the effectiveness of the plan, communication to the physician when changes are needed or outcomes are not appropriate
• Continuous assessment of resident needs with updates to appropriate individuals as it relates to care, services and changes in condition
• Implement measures to ensure resident wellness and safety and the appropriate documentation of those measures
• Work in a team fashion to meet resident needs
• Respond to emergencies timely and in a calm and competent manner
• Report incidents and changes in resident condition to Healthcare Director and with the appropriate documentation
• Be an active part of all activities including community events
• Be present and assist with the delivery of meals in the dining room, monitoring resident acceptance of diets and assuring adequate nutrition
• Maintain high standards with regard to record keeping, resident and staff documentation
• Establish and maintain a positive working relationship with all departments
• Identify risk factors for residents and initiate interventions including the appropriate documentation of that intervention
• Provide a thorough report of resident status to on-coming nursing team, at each change of shift, and in the Daily Log
• Provide motivation and direction to CNA/RMA’s
• Must be certified or willing to take and pass the certification classes required by the state in which employed
• Attend required trainings, CEU’s and staff meetings
• Adhere to and implement all policies and procedures
• Practice proper body mechanics and safe resident transferring techniques
• Adhere to attendance policies; report to work on time and prepared to begin duties
• Complete all other assigned duties

Requirements

• Current LPN license to practice nursing in the state in which the community is located, holding a license in good standing with the Board of Nursing
• Experience working with the senior population, preferably in assisted living or LTC
• Must be a team player who works well with others and has experience supervising staff
• Must possess critical thinking skills, ability to effectively communicate verbally and in writing
• Demonstrated ability to manage time efficiently with strong organizational skills
• Must be in good physical condition and be able to sit, stand, bend, kneel, walk, and lift up to 50 lbs
• Must be willing to work an 8-16 hour flexible schedule to include weekends and holidays
• Must attend all required trainings, CEU’s and staff meetings
• Must be certified or willing to take and pass the certification class required by the state in which employed
