Job summary

Lexington Premier Nursing & Rehab is seeking a dedicated Licensed Practical Nurse to join our team on the night shift. You will assess resident needs, provide direct care, administer medications, and work with RNs, CNAs, physicians and therapists to ensure high-quality outcomes for our residents.

The role requires Kentucky LPN licensure, graduation from an accredited LPN program, CPR/BLS certification, and strong communication skills.

Qualifications

• LPN licensure in Kentucky (valid).
• Graduation from an accredited LPN program.
• Experience in skilled nursing facility or similar healthcare setting preferred.
• Current CPR/BLS certification required; other state regulations may apply.
• Excellent verbal and written communication; ability to work with diverse teams.

Responsibilities

• Provide direct nursing care to residents; administer medications and treatments as prescribed.
• Monitor residents’ vital signs, document progress and report changes.
• Assist in development and implementation of individualized care plans.
• Maintain accurate documentation and communicate with residents, families and healthcare team.
• Adhere to facility policies, infection control and safety protocols.
