Job summary

The Citadel of Sterling is seeking a Licensed Practical Nurse (LPN) to join our team. You will care for a group of residents, implementing and evaluating the plan of care under physician guidance.

We offer a $2,500 sign-on bonus for full-time LPNs on Second or Third Shift, and a comprehensive benefits package. Prior LTC experience and a valid Illinois LPN license are preferred. Join us to contribute to high-quality, resident-centered care.

Qualifications

• Nursing degree or diploma from an accredited college/university.
• Current license to practice in the state as an LPN.
• 1 year of experience preferred.
• Long-term care experience preferred.

Responsibilities

• Provides licensed care to residents as ordered by physician and per standards and regulations.
• Performs resident assessments/observations and interventions; reviews care plans.
• Takes and records vitals; performs catheter care and IV therapy within license limits; provides respiratory care.
• Documents care with nurse's notes reflecting care and resident response.
• Maintains a safe environment; addresses safety risks promptly.
• Monitors and directs non-licensed ancillary services to maintain high care standards.
• Reports information to residents, families, physicians, guardians, and facility staff as required.
• Ensures care plans are implemented and suggests changes to the IDT when needed.
• Ensures infection control standards are followed and non-compliance is addressed.
• Adheres to HIPAA policies and company procedures.
• All other duties as assigned.
