Citadel Healthcare in Sterling, IL is seeking a Licensed Practical Nurse (LPN) to join our Second or Third Shift team. You will care for a group of residents, performing assessments, implementing and evaluating care plans, and coordinating with the IDT.

This is a full-time position with a $2,500 sign-on bonus and a focus on safe, compassionate care. We value excellent communication, the ability to multi-task under pressure, infection control, and a flexible schedule.

Qualifications

  • Nursing degree or diploma from an accredited College or University.
  • Current license to practice in the State as a Licensed Practical Nurse.
  • 1 year of experience preferred.
  • Long-Term care experience preferred.

Responsibilities

  • Provides licensed care to residents as ordered by physician and per standards.
  • Performs resident assessments/observations and updates care plans.
  • Take vital signs, assist with IV therapy and respiratory care when appropriate.
  • Document care and resident responses accurately in records.
  • Maintain a safe environment and address safety risks promptly.
  • Coordinate with non-licensed staff to meet care standards.
  • Communicate with residents, families, physicians and staff as required.
  • Implement and maintain care plans; propose changes as needed.
  • Ensure infection control compliance and HIPAA adherence.

Job description

We are offering a $2,500 sign-on bonus for full-time Licensed Practical Nurses joining our Second or Third Shift teams!

The Citadel of Sterling is seeking a Licensed Practical Nurse (LPN) to join their team! As a Licensed Practical Nurse (LPN), you will be caring for a group of residents and assuming responsibility and accountability for them. Nursing is provided through assessment, implementation, and evaluation of the plan of care.

Licensed Practical Nurse (LPN) Duties and Responsibilities
  • Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations.

  • Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care.

  • Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments.

  • Charts nurse’s notes in an informative and descriptive manner that reflects the care provided aswell as the resident’s response to the care.

  • Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted.

  • Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care.

  • Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines.

  • Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards.

  • Ensures compliance with infection control standards. Immediately corrects/addresses identified instances of non-compliance.

  • Follows and adheres to company policies and procedures regarding HIPAA (Health Insurance Portability and Accountability) compliance.

  • All other duties, as assigned.

Licensed Practical (LPN) NurseQualifications and Skills
  • Excellent written and verbal communication skills in English.

  • Ability to multi-task and remain calm under stress.

  • Analytical and problem-solving skills.

  • Understands and follows infection control standards, policies and procedures.

  • Must be able to adapt to a flexible work schedule and occasional overnight travel.

  • Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff.

Licensed Practical Nurse (LPN) Education and Experience
  • Nursing Degree or diploma from an accredited College or University.

  • Possess a current license to practice in the State as a Licensed Practical Nurse.

  • 1 year of experience preferred.

  • Long-Term care experience preferred.

Physical Requirements
  • Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.

  • Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
