Care Comfort Hospice is seeking Part Time/PRN LPN Hospice Nurses in Pennsylvania. The role requires an LPN license in good standing and typically 1–2 years in acute care or 1 year in home health/hospice. The nurse will provide hands-on hospice care, coordinate symptom management, and support families within the interdisciplinary team.
Orientation and on-call responsibilities are part of the schedule. The position emphasizes comfort, dignity, and education for patients and families, with
Skilled Nurse from an Accredited program currently licensed and in good standing with the Pennsylvania State Board of Nursing. One to 2 years’ experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required.
It is the responsibility of the Visit Nurse to coordinate and provide the highest level of hospice care to patients and families.
The Visit Nurse works in conjunction with the Nurse Case Manager, Director of Nursing (DON) and the Interdisciplinary Team to:
Care Comfort Hospice will provide orientation and training, as necessary, to assist the Hospice Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file.
The Hospice Nurse will be on call according to scheduling. During the hours when on call, the Visit Nurse is expected to follow On Call Protocol and to respond to all pages within 20 minutes.
Reports directly to the Nurse Case Manager and Director of Nursing.
