Angels Care Home Health is seeking an LPN Scheduler to manage scheduling of nursing visits using Home Care Home Base software. The role requires strong computer, communication, and organizational skills to coordinate care across the interdisciplinary team.

Competenze

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LVN/LPN.
• Current State’s Driver’s License.
• A reliable source of transportation and valid/current auto liability insurance is required.

• Schedule and appropriately document patient schedules in a timely manner.
• Coordinate care with management and the patient interdisciplinary team.
• Provide effective communication to patients, staff, other health care professionals and referral sources.
