Job summary

Interim HealthCare Inc. is seeking an LPN to provide at-home, patient-centered care in the North Sarasota and Bradenton areas. The role emphasizes 1:1 nursing with a focus on dignity, safety and independence, with overnight and day shifts available.

Our team supports professional growth through online training and CEU opportunities. You'll perform vitals, assessments and progress documentation, coordinate with an RN supervisor, and educate families on plans of care and home safety.

Qualifications

• Graduate of an accredited practical nursing program and active LPN license in Florida.
• Minimum of 1 year of LPN experience, ideally in home healthcare.
• CPR certification, TB test, Background check.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing, strong communication skills and compassion.

Responsibilities

• Provide fundamental home-based care to patients with chronic illness or disease.
• Take patient vitals, do assessments, document progress and report changes to RN supervisor.
• Educate family on the patient’s plan of care, medication and home safety.
