Typical Responsibilities:

• Follow individual service plans; administer or supervise treatments.
• Make rounds, record visits, and work with physicians.
• Administer injections, perform testing, record vital signs.
• Document resident episodes or status changes.
• Collaborate with staff on activities and care.
• Manage medications: order, administer, chart.
• Complete assessments, incident reports, and discharge documentation.
• Supervise residents self-administering medication; assist with daily activities.
• Other duties as assigned.

Qualifications:

• Licensed LPN or RN.
• Ability to lift 51 lbs and assist residents physically.
• Fluent in English.
• Experience with memory care residents preferred if applicable.
