Job summary

Phoenix Home Care & Hospice is hiring experienced home health nurses to deliver compassionate, patient-focused care in patients’ homes. You will administer medications, monitor conditions, perform treatments, and educate families while coordinating with our clinical team.

Qualifications

• Active Missouri RN or LPN license (or Compact license) required.
• Strong assessment and communication skills essential.
• Compassionate heart, dependable character, and self-motivated spirit.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Provide one-on-one nursing care in patients' homes.
• Administer and manage medications.
• Monitor and document patient conditions.
• Perform treatments as ordered by the physician.

• Collaborate with the Phoenix clinical team to ensure high-quality care.
