Job summary

Phoenix Home Care & Hospice in Missouri is seeking qualified nurses to provide one-on-one care in patients' homes, administering medications, monitoring conditions, and performing physician-ordered treatments.

Qualifications

• Active Illinois RN or LPN license (or Compact license).
• Strong assessment and communication skills.
• A compassionate heart, dependable character, and self-motivated spirit.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide compassionate, one-on-one nursing care in the comfort of your patients' homes.
• Administer and manage medications.
• Monitor and document patient conditions.
• Perform treatments as ordered by the physician.
• Educate, encourage, and support family members.
• Work alongside the Phoenix clinical team to provide exceptional care.
