Job summary

Phoenix Home Care and Hospice is seeking LPNs and RNs to deliver compassionate, in-home pediatric and young adult nursing. You’ll provide focused, one-on-one care, build meaningful family relationships, and help patients thrive at home with tailored treatment plans.

Qualifications

• Current and active LPN or RN license.
• Pediatric experience preferred; training provided.
• Patient-centered care mindset and strong communication skills.

Responsibilities

• Provide one-on-one skilled nursing care in patients' homes.
• Develop and adjust care plans with families and caregivers.
• Educate families on at-home care techniques, safety, and monitoring.
