Job summary

Fairpark Health and Rehabilitation is seeking licensed practical nurses and registered nurses to join a mission-driven team dedicated to residents and staff. You will coordinate with the healthcare team to assess and implement patient care plans, administer medications, monitor vital signs, and participate in discharge planning in a long-term care setting.

Qualifications

• Current RN or LPN license in the state.
• At least one year of LPN or RN experience in a long-term care setting preferred.
• Knowledge of MatrixCare is preferred.

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans.
• Prepares and administers medications and records prescribed treatments.
• Records patients' medical information and vital signs.
• Monitors equipment and interprets electronic displays.
• Participates in discharge planning and provides bedside care.
