Job summary

SNF DIVISION is seeking a compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our day shift nursing team. The ideal candidate will provide skilled, resident-centered care that promotes dignity, independence and quality of life.

Responsibilities include administering medications and treatments, assessing residents, documenting accurately, communicating with physicians and families, developing and updating individualized care plans, and supervising CNAs.

Qualifications

• Must be a licensed LPN or RN.
• Provide skilled nursing care and administer treatments as prescribed.
• Accurate and timely documentation and communication with care team.

Responsibilities

• Deliver skilled nursing care to residents.
• Assess and monitor residents' health status.
• Administer medications and treatments as prescribed.
• Maintain accurate documentation of nursing care.
• Communicate with physicians, families and interdisciplinary team.
• Develop and update individualized care plans.
• Supervise CNAs and ensure infection control practices.
• Assist with admissions, discharges and transfers.
• Support wound care and physician orders.

Additional responsibilitiesinclude supervising andsupporting Certified NursingAssistants (CNAs), ensuringcompliance with infectioncontrol practices andsafety protocols, completingadmissions, discharges,and transfers asneeded, and maintainingcompliance with stateand federal regulations, facility policies, andnursing standards. Youwill assistwith wound care, medication management,physician orders, andresidenteducation while fosteringa positive, respectful, and team-orientedwork environment. Thesuccessful candidate willdemonstrate excellent clinicaljudgment, strong organizationalandcommunication skills, theability to prioritizemultiple responsibilitiesin a fast-paced environment, anda genuine passionfor delivering compassionate, high-quality care.
