Job summary

Qualifications

• Requires LVN/LPN or RN license.
• High school diploma or GED or equivalent.
• Knowledge of nursing theory and clinical practice.
• Ability to lift at least 51 lbs and assist residents.
• English language proficiency (read, write, speak).

Responsibilities

• Follow individual service plans and administer prescribed treatments.
• Make rounds with service plans and physicians; document visits.
• Administer injections and perform in-house testing; record vital signs.
• Document acute episodes or significant changes in resident status.
• Collaborate with Activity Director, DON, and Nurse Aides in carrying out activities.
• Receive or place calls to physicians and follow through with orders.
• Prepare and administer medications; chart accurately.
• Order medications and refills; manage pharmacy documentation.
• Chart shifts per facility policy and guidelines.
• Supervise self-administered medications; assist with daily living activities.
• Provide emotional and social support to residents; maintain confidentiality.
• Inform supervisor of resident issues; maintain safety and professional demeanor.
• Perform other duties as assigned.
