Job summary

Stonecreek Health and Rehabilitation in Kentucky Paducah is seeking an LPN or RN for our MDS Coordinator position. We value our staff and provide training and ongoing support to help you excel in this essential role.

You will work with an interdisciplinary team to ensure MDS/RAI accuracy, perform chart reviews, and support care planning. Current LPN/RN license in good standing is required; LTC experience is preferred but not mandatory.

Qualifications

• Experience with MDS/RAI processes is preferred.
• Current license as LPN or RN in good standing is required.
• Long-term care experience is preferred.

Responsibilities

• Ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
• Observe direct nursing care, review documentation, and make appropriate recommendations.
• Assist with chart audits.
• Provide clinical support and direction to appropriate resource materials.
