Job summary

Mt. Pleasant Health and Rehabilitation is seeking an MDS Coordinator (LPN or RN) with MDS 3.0 certification to join our in-house team in Mount Pleasant, TN.

You will coordinate MDS/RAI with the interdisciplinary group, ensure regulatory compliance, and support accurate billing and care planning. Ideal candidates have long-term care experience and strong knowledge of Medicaid/Medicare requirements, with an eye for detail and organization to maintain timely documentation and high-quality resident

Qualifications

• Current license as LPN or RN
• Certification in MDS
• Previous MDS experience in Skilled Nursing Facilities
• Must have MDS 3.0 experience
• Knowledge of Medicaid and Medicare
• Long-term care experience preferred

Responsibilities

• Lead MDS/RAI processes with the interdisciplinary team
• Review documentation and recommend improvements; assist with chart audits
• Provide clinical support and reference materials to the team
• Ensure MDS documents meet federal/state guidelines and organizational policy
• Participate in daily PPS and weekly Medicare meetings to ensure billing compliance
• Assist with completing the Resident Assessment Instrument with the inter-disciplinary team

Responsibilities of the MDS Coordinator:

• Ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
• Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.
• Provide clinical support and direction to appropriate resource materials.
• Verify all MDS documents required either by Federal and/or State Guidelines or Organizational policy are easily available at all times is completed timely as set forth by Federal Regulations.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met.
• Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.
• Assist with the coordination and participate in the completion of the Resident Assessment Instrument in accordance with current Federal and State Regulations.

Requirements of the MDS Coordinator

• Current license as LPN or RN
• Certification in MDS
• Previous MDS experience gained in Skilled Nursing Facilities
• Must have MDS 3.0 Experience
• Knowledge of Medicaid and Medicare
• Must have long-term care experience
