Job summary

Pathways Healthcare, a leader in home health and hospice, seeks a per diem Registered Nurse to assess patients, write and revise care plans, and administer treatments in patients' homes. You will document care, coordinate with physicians, and participate in interdisciplinary teams to ensure quality outcomes.

The role offers a flexible schedule, mileage reimbursement, and a focus on patient-centered care in the Westwood area with a strong supportive culture.

Qualifications

• 2+ years of nursing experience, preferably geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient status and documents findings.
• Write and initiate plan of care; regularly re-evaluate patient and family needs.
• Revise the plan of care as necessary with the physician's guidance.
• Develop a care plan with goals based on nursing diagnosis and involve patient/family.
• Administer medications and treatments per physician orders.
• Complete and submit accurate clinical notes; track pain/symptom changes.
• Communicate with physicians about patient needs and changes in condition.
• Provide a safe home environment and participate in interdisciplinary meetings.
• Participate in on-call duties and ensure equipment/services are available.

Responsibilities:

Patient Care:

• Assesses and evaluates patient’s status by:


• Writing and initiating plan of care.


• Regularly re-evaluating patient and family/caregiver needs.


• Participating in revising the plan of care as necessary.


• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.


• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.


• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.


• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.


• Provides and maintains a safe environment for the patient.


• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.


• Ensures that arrangements for equipment and other necessary items and services are available.


• Supervises ancillary personnel and delegates responsibilities when required.


• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.


• Fulfills the obligation of requested and/or accepted case assignments.


• Actively participates in quality assessment performance improvement teams and activities.

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.

Benefits & Compensation:

• Pay Strucutre: Paid per visit ($50 weekdays / $55 weekends & holidays). Training, meetings, and other hourly activities are paid at $35/hr.
