Job summary

Crossroads Treatment Centers is hiring an LPN/RN for our outpatient MAT clinics. You will follow orders, identify patients, dispense medications, and coordinate care with the clinical team to ensure continuous patient support.

The role emphasizes accurate documentation, compliance, and empathetic patient interaction. Nursing license in-state or compact, 3+ years of experience, and CPR/BLS are preferred.

Qualifications

• Active LPN or RN license in the practicing state or a multi-state compact license.
• 3+ years of nursing experience preferred.
• Strong phlebotomy skills; blood draws on difficult veins a plus.
• Current CPR or BLS certification preferred.

Responsibilities

• Follow policies and procedures to correctly identify patients and administer medication per orders.
• Dispense daily and take-home medication at the dosing window.
• Coordinate care with clinical and medical staff for continuity of patient care.
• Maintain accurate patient documentation including assessing for impairment.
• Ensure medication counts are accurate daily.
• Follow compliance policies around documentation practices.
• Participate in nursing team meetings and cross treatment meetings.
• Connect with patients with empathy and a listening ear.
