Job summary

Crossroads Treatment Centers is seeking an LPN/RN for our Columbia, SC outpatient MAT program. You will identify patients, administer medications per orders, and coordinate care with clinical staff to ensure continuity of treatment.

This role emphasizes accurate documentation, adherence to regulatory policies, and a compassionate approach to patients with opioid use disorder. A valid nursing license and CPR/BLS are required or preferred.

Qualifications

• Licensed Practical or Registered Nurse license issued in the state where you will practice.
• 3+ years of nursing experience preferred.
• Strong phlebotomy skills; ability to draw blood on challenging veins is a plus.
• Active CPR or BLS certification preferred.

Responsibilities

• Identify patients and administer medications per provider orders.
• Dispense daily and take-home medication at the dosing window.
• Coordinate care with clinical staff for continuity of patient care.
• Maintain accurate patient documentation including impairment assessments.
• Ensure medication counts are accurate and updated daily.
• Follow compliance policies around documentation practices.
