Job summary

Crossroads Treatment Centers in Kentucky is seeking an LPN/RN to join our outpatient nursing team. Follow policies to correctly identify patients and administer medications per orders, and coordinate care with clinical staff for continuity of patient care.

You will maintain patient documentation, ensure accurate medication counts, and participate in nursing team meetings and cross-treatment meetings, all while offering empathy to patients and families.

Qualifications

• Licensed Practical or Registered Nurse license in the practicing state.
• License must be in good standing and active.
• 3+ years nursing experience preferred.
• Phlebotomy skills are a plus.
• Current CPR or BLS certification preferred.

Responsibilities

• Follow policies and procedures to identify patients and administer medication per orders.
• Dispense daily and take-home medication at the dosing window.
• Coordinate care with clinical staff for continuity of patient care.
• Maintain accurate patient documentation including assessment of impairment.
• Participate in nursing team meetings and cross-treatment meetings.
