Job summary

Crossroads Treatment Centers in Kentucky seeks a licensed practical nurse (LPN) or registered nurse (RN) to join our team on site. You will follow clinicians’ orders, correctly identify patients, dispense medications, and coordinate care with clinical staff to ensure continuity of patient care.

This on-site role offers a stable morning schedule with a small nursing team, excellent training, and daily opportunities to impact lives through MAT-based patient care.

Qualifications

• LPN/RN license in good standing in the practicing state.
• 3+ years nursing experience preferred.
• Strong phlebotomy skills; drawing blood from collapsed veins is a plus.
• Active CPR or BLS certification preferred.

Responsibilities

• Follow policies and procedures to correctly identify patients and administer medication per providers' orders.
• Dispense daily and take-home medication at the dosing window.
• Coordinate care with clinical and medical staff for continuity of patient care.
• Maintain and complete accurate patient documentation including assessing for impairment.
• Ensure medication counts are accurate and completed daily.
• Follow compliance policies and documentation practices and expectations.
• Participate in nursing team meetings and cross-treatment meetings.
• Connect with patients, offering empathy and a listening ear.

Day in the Life of a LPN/RN

• Dispense daily and take-home medication at the dosing window.
• Coordinate care with clinical and medical staff for continuity of patient care.
• Maintain and complete accurate patient documentation including assessing for impairment.
• Ensuring medication count is accurate and completed daily.
• Follow compliance policies and procedures around documentation practices and expectations.
• Participate in nursing team meetings and cross treatment meetings.
• Connecting with patients, offering true empathy and a listening ear to those struggling.

Education And Experience Requirements

• Licensed Practical or Registered Nurse License issued in the state in which they will be practicing or a multi-state compact status license.
  • License should be in good standing, unencumbered, and active. If applicable, disciplinary actions documented on any license will be reviewed on a case-by-case basis.
• 3+ years of nursing experience is preferred.
• Strong phlebotomy skills, experience drawing blood on collapsed veins is a plus.
• Active and current CPR or BLS certification is preferred.

Schedule

• Monday-Friday; 4:45am–1:00pm
• Rotating Saturday; 4:45am–8:30am

• Consistent schedule. Early in time and out time, no 12 hour shifts nor is there a 2nd or 3rd shift. You will also never be on call
• Small nursing team. Depending on patient census, teams can range anywhere from 2-12
