Job summary

Homecare with Heart is seeking compassionate LPNs or RNs to provide home care in Leetonia, Ohio. You will oversee quality care for clients and ensure their safety while promoting independence.

Starting pay is $26/hour for LPNs and $30/hour for RNs, with generous benefits including health insurance, PTO, and a $1,500 sign-on bonus. Join a caring team that has served the Tri-County area since 2004.

Qualifications

  • Valid State LPN/RN License required.
  • Must have a valid driver’s license and use of automobile.
  • Annual training as required by Ohio Medicaid and Home Care Program.
  • Ability to read and understand the Plan of Care and agency policies.

Responsibilities

  • Administer medication according to standards.
  • Encourage social skills and self-care.
  • Perform client treatments following the Plan of Care.
  • Document care provided and any changes in condition.
  • Collaborate with the clinical team for client care improvement.

LPN or RN –Every other week Monday 7a – 4p & Friday 7a – 6p

Homecare with Heart is offering you the most competitive pay scale in the area. We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. Starting rate for LPN: $26 per hour, RN: $30 per hour depending on experience.

Responsibilities

What You’ll Do:

  • Administer medication in accordance with standards of practice.
  • Encourage social/rehabilitative skills and encourage self‑care and independence.
  • Perform client treatments in accordance with Plan of Care.
  • Assist the consumer in learning appropriate self‑care techniques.
  • Prepare clinical and progress notes to document care provided.
  • Monitor, document accordingly and report any change in client condition to Clinical Director or Clinical Supervisor.
  • Promote activities to achieve a safe and healthy environment.
  • Observe aseptic technique when performing client procedures or care.
  • Collaborate with other members of the clinical team to develop and improve the Plan of Care.
  • Assist the client in achieving established goals in the Plan of Care developed by the RN.
  • Maintain productivity according to the standards of the agency.
  • Maintain license, CEUs and mandatory employment requirements.
  • Participate in client care meetings, in‑services, orientation and staff meetings.
  • Provide service in accordance with agency’s philosophy, standards, policies and scope of practice.
  • Participate in Performance Improvement and adhere to HIPAA guidelines.
  • Other duties as assigned.
Qualifications

What You’ll Bring to the Team:

  • Valid State LPN/RN License.
  • Valid driver’s license and use of automobile is required due to travel involved.
  • Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities.
  • Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
  • Must be able to use a computer, laptop or tablet.
  • Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
  • Employees must promote excellent customer service and always provide exceptional client care, while staying within the rules of the agency and rules of each program served.
