Job summary

Acacia Network seeks an LPN to provide medical care in a residential treatment program for substance use disorders in New York. Under supervision, you will assess health histories, perform exams, monitor meds, and coordinate care with physicians and the interdisciplinary team.

You will document all nursing activities in the EHR, participate in staff development, and support HIV/AIDS prevention and education initiatives. A commitment to quality and cultural sensitivity is essential.

Qualifications

• Must hold an active NY LPN license and BLS certification.
• Experience in health care facilities with strong documentation.
• Familiarity with infection control practices and OTP systems.
• Ability to coordinate care with physicians and staff.
• Excellent communication and organizational skills.

Responsibilities

• Act as health coordinator under the program director.
• Review medical histories submitted by intake.
• Assist referrals to health care providers.
• Manage withdrawal symptoms in residential stabilization.
• Document medications in the electronic health record.
• Coordinate with primary care physicians for treatment.
