Northeast Arc in Topsfield, MA is seeking a Licensed Practical Nurse (LPN) to join our residential care team. You will provide direct care, follow the plan of care, monitor health status, and report changes to the on-call RN while collaborating with residential staff to support individuals in our community residence.

Major responsibilities include health monitoring, medication management, care plan documentation, medical coordination, and compliant, professional communication with teams.

Formación

• Licensed LPN in Massachusetts with minimum of 1 year of experience.
• Experience working with individuals with intellectual or developmental disabilities preferred.
• Must maintain CPR certification.
• Must have a valid driver’s license and reliable transportation.

Responsabilidades

• Health monitoring and care, including vital signs and symptom checks.
• Medication administration and documentation per MAP regulations.
• Care plan documentation and routine paperwork.
• Assist with scheduling medical appointments and accompany individuals as needed.
• Provide shift-to-shift reports and maintain compliance with safety standards.
