Qualifications

• Current LPN license in Kentucky (or ability to obtain in KY).
• Strong clinical nursing skills and attention to detail.
• Excellent communication and interpersonal abilities.
• Long-term care experience preferred, but not required.

Responsibilities

• Provide direct nursing care to residents per care plans.
• Administer medications and treatments as prescribed.
• Monitor residents' health status and report changes to the care team.
• Collaborate with CNAs, RNs, and interdisciplinary team members.
• Maintain high standard of care focused on dignity and comfort.
• Maintain accurate and timely documentation.
