Job summary

Health, Inc is seeking a Licensed Practical Nurse to provide essential nursing care, assess residents on admission, and monitor conditions across shifts. You will communicate significant changes to the interdisciplinary team and document assessments accurately, while assisting with ADLs and IV therapies as needed.

In this role you will administer medications, participate in care planning, and respond to emergencies.

Qualifications

• Must hold an LPN license and be licensed in Missouri (and Illinois if applicable).
• Current CPR/AED and IV certification are required.
• Minimum 2 years, preferably 3 years, in long-term care and rehab settings.
• Strong communication, time management and organizational abilities.

Responsibilities

• Assess and monitor residents' health status.
• Communicate changes to the IDT and physicians; document assessments and interventions.
• Administer medications, treatments, and IV fluids as prescribed.
• Assist residents with ADLs and participate in care planning updates.
• Respond to emergencies and maintain continuing education requirements.
