Job summary

Qualifications

• Nursing degree from an accredited college or university.
• Graduate of an approved LPN/LVN program.
• Current unrestricted LPN license in practicing state.
• Current CPR certification.

Responsibilities

• Direct the daily activities of the LNA’s in accordance with current regulations and facility policies.
• Ensure policies and procedures are followed by nursing personnel.
• Participate in admission, discharge and transfers of residents as required.
• Observe changes in residents’ status and notify physician and family.
• Transcribe physician orders and carry out orders as written.
• Prepare and administer medications as per orders and observe adverse effects.
• Perform wound treatments and document observations.
• Schedule follow-up appointments and transportation as needed.
• Provide nursing leadership to assigned staff.
• Provide direct care such as colostomy changes, tube feedings, wound care, suctioning, IV administration as required.
• Collaborate with interdisciplinary team and file grievances as necessary.
• Update care plans and participate in survey and end-of-life care as required.
• Ensure medications, supplies, and equipment are stocked.
• Maintain confidentiality of protected health information.
