Job summary

Eastern Health Group seeks an LPN to oversee direct nursing care for residents. Under RN/physician supervision, you will ensure high standards of care in line with state regulations and facility policies.

Responsibilities include rounds, care plan implementation, documentation, and collaboration with CNAs. Requires an active LPN license and CPR certification; one year of experience preferred. On-site position with sign-on bonus.

Qualifications

• Current LPN license in good standing.
• Must have CPR certification.
• Excellent written and verbal English communication.
• One year related experience preferred.
• Proficient in Word/Excel; able to use Outlook.

Responsibilities

• Provide direct nursing care under RN or physician supervision.
• Oversee care delivery to residents and ensure policy compliance.
• Document services rendered and admission/discharge notes.
• Monitor resident status with daily rounds and implement care plans.
• Maintain infection control and a safe working environment.
• Coordinate with CNAs to ensure smooth workflow.
