EASTERN HEALTH GROUP is seeking a Licensed Practical Nurse (LPN) to oversee care delivery for residents, providing direct nursing care under supervision of RN or physician in accordance with facility policies and state regulations.

Qualifications

• Licensed practical nurse with current state license.
• One year related experience preferred.
• Current CPR certification.
• Excellent documentation and technical skills.
• Effective verbal and written English communication skills.
• Proficient with MS Word, Excel, PowerPoint and Outlook.
• Ability to maintain confidentiality and work under pressure.
• Team player with strong organizational skills.

Responsibilities

• Provide direct nursing care under supervision of RN or physician.
• Participate in discharge planning and resident evaluations.
• Document services rendered to residents.
• Coordinate with CNAs to ensure care flow.
• Write or review admission notes and chart essential information.
• Conduct daily rounds to monitor resident care and status.
• Implement care plans in accordance with physician orders.
• Follow nursing policies and procedures.
• Promote positive relationships with staff and residents.
