Job summary

Viva Senior Living at Lebanon is seeking a Licensed Practical Nurse to provide compassionate, high-quality nursing care to our residents in the Clinical department.

You must hold a valid LPN license, a high school diploma or GED, and be able to read, write, speak and understand English. You will coordinate care plans, administer medications, and respond to emergencies to ensure resident well-being.

Qualifications

• Must possess a valid LPN license in good standing from an approved State School of Nursing.
• Must possess at least a high school diploma or GED.
• Must be able to read, write, speak, and understand English.
• Must adhere to attendance policies and facility procedures.

Responsibilities

• Coordinate the day-to-day care of residents per the service plan.
• Know the individualized care plan and support residents accordingly.
• Coordinate services with hospice, home health, and pharmacies.
• Coordinate medication management and document treatments per orders.
• Report changes in resident condition to the team and primary care providers.
• Assist with routine activities of daily living and basic nursing procedures.
