Job summary

Viva Senior Living in Harleysville, PA is seeking a Licensed Practical Nurse to join the Clinical Department. The LPN will coordinate day-to-day resident care, support care plans, and collaborate with the care team to ensure compassionate, compliant service.

Qualifications

• Must possess a valid LPN license in good standing in accordance with laws of this state from an approved State School of Nursing.
• Must possess, at a minimum, a high school diploma or a GED.
• Must be able to read, write, speak, and understand English.
• Must adhere to Viva Senior Living’s attendance policies and follow the terms and conditions of employment as set forth in the Employee Handbook.

Responsibilities

• Coordinate the day-to-day care of residents in accordance with the service plan, coordinating staff and supervising RMAs and direct care staff.
• Provide support to residents according to their care plan and contribute to care planning with nursing staff.
• Coordinate services with hospice, home health, and durable medical equipment providers as ordered.
• Administer and document medication and treatments per physician’s order and record all care provided.
