Shalom West is a Memory Care community in Colorado, seeking an experienced Resident Care Coordinator to support the Health and Wellness Director. You will assist residents with ADLs, medication administration, and daily living activities, while maintaining compassionate, resident-centered care.

Qualifications

• Must be a graduate of an approved LPN program.
• Current Colorado LPN license.
• Minimum 1 year related experience.
• BLS/CPR certification required.

Responsibilities

• Monitor residents' health status and report changes promptly.
• Maintain accurate resident records and documentation.
• Coordinate care with physicians, pharmacies, and hospitals.
• Assist with admissions, orientation, and transition into the community.
• Supervise Caregivers and QMAPs as assigned.
