Job summary

HealthBridge is hiring an LPN to monitor patients with chronic conditions using remote patient monitoring (RPM) and care coordination tools. You will engage patients regularly, assess needs, and coordinate with NPs and primary care providers while documenting interactions in EMR systems.

This role emphasizes preventive, data-driven care over bedside tasks, with a focus on improving patient outcomes and reducing hospitalizations.

Qualifications

• Active Virginia LPN license (preferred) or ability to obtain quickly.
• Strong clinical judgment and clear, professional phone communication.
• Highly organized and comfortable working across multiple platforms.
• Experience with RPM, telehealth, or care coordination preferred.
• Interest in preventive care and patient engagement.
• Ability to work effectively in a fast-paced, structured environment.

Responsibilities

• Monitor RPM dashboards and respond to alerts (blood pressure, glucose, weight, etc.).
• Call patients regularly to assess symptoms and reinforce device usage.
• Coordinate follow-up with NPs, primary care providers, and caregivers.
• Document patient interactions in EMR and internal systems.
• Troubleshoot device issues to ensure patient compliance.
• Serve as a primary point of contact for a panel of patients.
• Support patient enrollment and onboarding as needed.
• Collaborate daily with NPs, intake team, and support staff.

What You’ll Do

You will play a key role in monitoring and supporting patients managing chronic conditions—engaging them consistently and coordinating care when it matters most.

• Monitor RPM dashboards and respond to alerts (blood pressure, glucose, weight, etc.)
• Call patients regularly to assess symptoms and reinforce device usage
• Coordinate follow-up with NPs, primary care providers, and caregivers
• Document patient interactions in EMR and internal systems
