Job summary

Lifepoint Health®—Texas Rehabilitation Hospital of Fort Worth is seeking a Licensed Practical Nurse to provide direct patient care on a full-time Day shift (7a-7p). You will assess patients, implement nursing interventions, and educate patients and families throughout rehabilitation.

Collaboration with an interdisciplinary team is essential to deliver high-quality care. Applicants should have an accredited LPN program, active LPN license, CPR/BCLS, IV certification, and 1+ year of LPN experience

Qualifications

• Graduate of accredited licensed practical/vocational program.
• Current state licensure as Licensed Practical/Vocational Nurse.
• CPR/BCLS certification; ACLS preferred.
• IV Certified required or obtained within 6 months highly preferred.
• Minimum one year LPN experience required and six months Medical/Surgical experience in an acute care setting preferred
• Excellent oral and written communication and interpersonal skills

Responsibilities

• Accurately performs patient assessments and identified patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to the condition and age of the patient.
• Works cooperatively and actively participates with other members of the interdisciplinary team.
