St. Mary's Regional Medical Center in Enid, OK is seeking a dedicated LPN to join our Acute Rehab team on a PRN basis. You will provide high‑quality nursing care in collaboration with a registered nurse and multidisciplinary team.

Responsibilities include participating in care plan modification, evaluating patient responses, reassessment, focused assessments, data collection and documentation. Requires Oklahoma LPN license and BLS.

Qualifications

  • Graduate of an accredited LPN program.
  • Current LPN license in Oklahoma.
  • BLS (current upon hire).
  • Venipuncture skills required.
  • One year of full time or three years part time experience in acute care setting preferred.
  • De-escalation training (TEAM) completed within 6 months of hire.
  • Basic computer skills.

Responsibilities

  • Participates in the modification of the plan of care by collaborating with other members of the multidisciplinary team with consideration of the developmental needs of the patient.
  • Participates in the evaluation of the patient’s response to interventions based upon predicted outcome.
  • Reassesses the patient as defined in the Nursing Standards of Care Policy and modifies the plan of care as needed.
  • Contributes to the assessment of the patient by conducting focused assessments of the patient.
  • Collects data and compares the data collected to the patient’s previous condition and documents/reports as appropriate.

Summary Of Job

Performs the functions of LPN in implementing and evaluating nursing care for all assigned patient care in collaboration with a registered nurse. Practices nursing in accordance with established standard of practice, policies and procedures.
