Oklahoma Ag is seeking an LPN Provider Services Coordinator at the Claremore Veterans Home to support rounds preparation, order coordination, and clinical information flow. This role involves assisting providers with exams, coordinating tests, and ensuring documentation is accurate and timely.

The position requires an active Oklahoma LPN license, strong communication, and basic EMR skills. It is a full-time, on-site role with duties aligned to facility policy and patient care excellence.

Formación

• Current, unencumbered Oklahoma LPN license.
• Effective English reading/writing to understand clinical instructions and policies.
• Basic EMR/computer skills.
• Strong communication and teamwork abilities.
• Ability to maintain required training and competency.

Responsabilidades

• Prepare and organize weekly provider-round resident lists and records for rounds.
• Assist with provider-requested letters, forms, and documentation; route reports into EMR.
• Coordinate diagnostic testing and imaging requests; route reports to providers.
• Assist with resident examinations and minor procedures as directed.
• Enter verbal/telephone orders only when permitted by policy and read-back.
• Coordinate admission orders and diagnostic needs; notify care team.
• Track QA data and communicate overdue items to leadership.
