Job summary

Complete Care - Connecticut is hiring an LPN to provide skilled nursing care to residents recently discharged from hospital, as well as long-term care patients. The role includes administering medications, wound care, monitoring health status, and collaborating with the care team to implement individualized plans.

Qualifications

• Must possess a current LPN license in the state of Connecticut.
• Professional and clinically skilled with leadership qualities.
• Able to pass background check, drug screen, and references.

Responsibilities

• Administer medications (oral, topical, IV, injectable).
• Perform wound care and prevention; manage pain and dressings.
• Monitor health status and analyze data for changes.
• Provide catheter/ostomy care and collect specimens.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers and others.
• Educate residents and families about health status and care expectations.
• Maintain detailed health records and care interactions.
• Oversee and delegate CNA duties to ensure quality care.
