Job summary

TN Quality Care Hospice in Clarksville is seeking a PRN LPN to join our compassionate team. This role offers flexible hours, weekend coverage, and the chance to deliver high-quality patient care.

You will provide direct nursing care under the State Nurse Practice Act, document care, educate patients and families, and participate in interdisciplinary coordination. Qualifications include LPN/LVN license, CPR, and a valid driver's license.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience (community health/hospice or medical-surgical preferred).
• Currently licensed as an LPN/LVN in the practicing state.
• Maintains a current CPR certification.
• Must have a valid driver’s license with insured vehicle.

Responsibilities

• Provides direct patient care as defined in State Nurse Practice Act.
• Implements current nursing practice following the plan of care.
• Provide accurate and timely documentation of patient services reflecting the plan of care.
• Assess and provide patient and family education pertinent to diagnosis and self-care plan.
• Participates in coordination of hospice services, reporting needs to the interdisciplinary group.
• Provides appropriate pain and symptom management.
• Evaluates patients’ response to treatments/medications.

What Youll Do

• Provides direct patient care as defined in State Nurse Practice Act


• Implements current nursing practice following a comprehensive assessment and the plan of care


• Provide accurate and timely documentation of patient services to reflect the plan of care


• Assess and provide patient and familycaregiver education and information pertinent to diagnosis and self plan of care


• Participates in coordination of hospice services appropriately reporting the identified needs to the interdisciplinary group Examples include hospice aide OT PT MSW Dietitian or Clinical Supervisor


• Provides appropriate painsymptom management


• Evaluates patients response to treatmentsmedications

Qualifications
