As a Weekend LPN you will triage weekend calls, provide direct patient care, document per the plan of care, and coordinate with weekend clinicians to ensure continuous patient support across the coverage area.

Qualifications

• Graduate of an accredited LPN or LVN program.
• Three years nursing experience.
• Hospice or medical-surgical experience preferred.
• Current LPN/LVN license in TN.
• CPR certification maintained.
• Valid driver’s license with insured vehicle.

Responsibilities

• Triages all calls through the weekend and liaises with weekend clinicians.
• Provides direct patient care as defined by the State Nurse Practice Act.
• Implements current nursing practice following assessment and the plan of care.
• Documents patient services accurately and timely to reflect the plan of care.
• Educates patient and family/caregiver on diagnosis and self-care plan.
• Coordinates hospice services and reports needs to interdisciplinary group.
• Delivers pain and symptom management and evaluates treatment responses.

What Youll Do

• This position will be responsible for triaging all calls through the weekend and liaising with other weekend clinicians as needed
• Provides direct patient care as defined in State Nurse Practice Act
• Implements current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and familycaregiver education and information pertinent to diagnosis and self plan of care
• Participates in coordination of hospice services appropriately reporting the identified needs to the interdisciplinary group
• Examples include hospice aide OT PT MSW Dietitian or Clinical Supervisor
• Provides appropriate painsymptom management
• Evaluates patients response to treatmentsmedications

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program
• Three years nursing experience
• Community healthhospice or medicalsurgical experience is preferred
• Currently licensed as an LPNLVN in the practicing state
• Maintains a current CPR certification
• Must be a licensed driver with an automobile that is insured in accordance with state andor organization requirements and is in good working order
