Wellpath is seeking a Licensed Practical Nurse (LPN) for PRN/Days in Kentucky to provide high-quality patient care under RN supervision. The role emphasizes assessment, medication administration, and collaboration with the care team in a structured setting.

Requirements include nursing licensure in the state, CPR certification, and a preference for at least 1 year of clinic experience. This position is available to United States residents and offers opportunities for ongoing training and growth

Qualifications

• Graduate from an accredited school of nursing.
• Current licensure as a Licensed Practical/Licensed Vocational Nurse in the state of employment.
• CPR certification required.
• Prefer a minimum of one (1) year clinic experience.

Responsibilities

• Under RN supervision, assesses patients' physical, psychological, and social dimensions and contributes to treatment planning.
• Administers medications and IV therapy (if certified) and performs diagnostic tests per protocols.
• Assists in controlled substance counts and responds to health emergencies per guidelines.
• Documents nursing encounters using SOAP charting and communicates with staff and physicians.
• Attends mandatory meetings and training to maintain high-quality patient care.
