Job summary

Onyx Care of Winnfield is seeking an LPN-PRN to join a compassionate team in a post-acute rehabilitation setting. You will provide skilled nursing care, support recovery goals, and build meaningful relationships with patients and families.

• Provide skilled nursing care to patients in a post-acute rehabilitation setting
• Deliver individualized care plans tailored to each patient's unique needs and recovery goals
• Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals
• Monitor patients' health conditions and report any changes or concerns to the healthcare team
• Collaborate with other healthcare professionals to ensure seamless patient care and support
• Maintain accurate and detailed patient records and documentation
