• Availability for per-diem / PRN shifts.

Responsibilities

• Assess patients on admission and during treatment.
• Provide regular patient status updates.
• Assist in implementing treatment plans under RN direction.
• Document accurately and timely.
• Supervise/administer/assist with medications.
• Provide direct resident care as needed.
• Follow and maintain plan of care for residents.
• Report to the Director of Nursing.

Job Duties:

• Conduct thorough assessments of each patient's medical status upon admissions and throughout the course of treatment.
• Provide regular patient status updates.
• Assist in implementing patient's treatment plan under direction of the Registered Nurse.
• Complete required documentation in an accurate and timely manner.
• Supervise, administer, and/or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Ensure the residents' care standards are maintained.
• Report to the Director of Nursing.

Qualifications:

• Valid LPN license in the State of PA
