Job summary

Maxim Healthcare Services, Inc. is seeking a Licensed Practical Nurse (LPN) to provide care to pediatric and/or adult patients in their own homes in the Richmond area.

Starting pay ranges from $28 to $32.50 per hour, negotiable based on experience and skill level. Role includes nursing care planning, medication administration, and care coordination, with comprehensive benefits including health/dental/vision, HSA, life insurance, PTO, and a 401(k).

Qualifications

• Active LPN license in the state where you practice.
• Current BLS certification.
• 1 year of LPN experience preferred.

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient care in home settings.
• Initiates the plan of care and revises as patient status changes.
• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on diagnosis and goals.
• Intervenes for changes in patient condition and reports to physician or supervisor.
• Administers medications and treatments as prescribed and documents effectiveness.
• Reviews plan of care and program protocols and identifies education and discharge needs.

Responsibilities:

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
• Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements:

• Current Licensed Practical Nurse(LPN) License for the state in which the nurse practices.
• Current Health Certificate (per facility Licensed Practical Nurse(LPN) contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior Licensed Practical Nurse (LPN) experience preferred.
